If you do not hear from me within one week from the time you submit the destination form, look in your SPAM mail. If you can't find it, contact me at destination@QOVF.org. Once you have received your destination, please have your quilt on its way within 2 weeks. If this is NOT possible, please email me at destination@QOVF.org.
Note: If you are using a spam filter that requests that we complete a form in order to allow QOVF to communicate with you, please understand that due to volume and staff constraints we cannot complete forms in the process of requesting quilt destinations.
Please first add email@example.com to your approved email addresses prior to making your destination request. If you do not add this email address to your approved email addresses, your request will be significantly delayed as we process requests that provide a quick response.

I use this form to find your QOV a destination. Only fill it out when you are 100% ready. 100% ready means your QOV has been or has:
washed (even if you pre-washed all your fabric)
Optional but highly desirable is the journal and pictures you may have taken.
