Requests of QOV for service members currently deployed in a combat zone will be deferred until the recipient’s return.
This form is used to assign your QOV to a facility on our Continuing Destinations List.
Only fill it out when you are 100% ready. Please read our Basic Instructions – 100% ready means you have completed the following:
- Washed (even if you pre-washed all your fabric)
- Presentation case
Optional but highly desirable is the journal and pictures you may have taken.
Quilts in patriotic colors of red, white and blue are always in high demand and preferred by our recipients, both male and female. There are a limited number of facilities on our continuing destinations list that request feminine quilts so the need is much smaller.
Please note: Completion of this form should only be done if you have a completed Quilt of Valor to send to a destination. DO NOT use this form to request a quilt for yourself or someone else. This can be done here: Request A Quilt.
If you do not hear from me within one week from the time you submit the destination form look in your SPAM mail. If you can’t find it contact me at destination@QOVF.org. Once you have received your destination please have your quilt on its way within 2 weeks. IF THIS IS NOT POSSIBLE please email me at destination@QOVF.org.
Note if you are using a spam filter: If you are using a spam filter that requests that we complete a form in order to allow QOVF to communicate with you, please understand that due to volume and staff constraints we cannot complete forms in the process of requesting quilt destinations. Prior to making your destination request please add karen.weber@QOVF.org to your approved email addresses. If you do not add this email address to your approved email addresses, your request will be significantly delayed as we process requests that provide a quick response.
